GenusPupClub

By signing below, I ("Owner") agree to the following terms with GenusPupClub ("Provider"):

1. CARE AUTHORIZATION. I authorize GenusPupClub and its employees to provide pet care services including but not limited to: walking, feeding, administering medications as directed, transporting to veterinary care in case of emergency, and making reasonable decisions regarding my pet's welfare.

2. HEALTH & VACCINATION. I confirm my pet is in good health, current on vaccinations (rabies, DHPP, bordetella), and free of contagious conditions. I will inform GenusPupClub of any changes in health status.

3. EMERGENCY VETERINARY CARE. In the event of illness or injury, I authorize GenusPupClub to seek veterinary care at my expense. I agree to reimburse all veterinary costs incurred on behalf of my pet. GenusPupClub will make reasonable efforts to contact me before seeking treatment.

4. LIABILITY WAIVER. I understand that pet care involves inherent risks. I release GenusPupClub, its owners, employees, and agents from liability for any injury, illness, damage, or loss arising from the care of my pet, except in cases of gross negligence.

5. PROPERTY ACCESS. If services are performed at my home, I grant GenusPupClub employees access to my property as needed to perform pet care services. I confirm there are no hazards that could endanger employees.

6. CANCELLATION POLICY. Cancellations must be made 24+ hours in advance for a full refund. Late cancellations (under 24 hours) incur a 50% charge. No-shows are charged in full.

7. PHOTOS & MEDIA. I grant GenusPupClub permission to take photos/videos of my pet during care and use them for business purposes (social media, website, marketing), unless I opt out below.

8. PAYMENT. I agree to pay for all services rendered at the agreed-upon rates. Payment is due upon completion of service or according to the agreed billing schedule.
